Indications of Quinolones
UTI

Ciprofloxacin is the only oral antibiotic that is affective against pseudomonas aeruginosa
MOD of Quinolones
Kill bacteria by inhibiting DNA synthesis.
Effective against gram-ve explaining their utility in GI and GU
Moxilfoxacin and levofloxacin are new drugs that have gram+ve activity making them able to treat LRTI
SE of Quinolones
generally well tolerated
GI upset
inflamation and rupture of muscle tendons
quinlones (especilally moxi) prolongs QT interval –> increased risk of arythmia
high risk of Cdiff
Warnings of Quinolones
pts with seizures
pts with risk factors of QT prolongation
Interactions of Quinolones
ciprofloxacin ihibits certain p450 drugs increasing risk of toxicity especially theophyline
dont use with drugs that prolong QT like macrolides
Tell me about Quinolones use
Usually kept for second or third line treatment because of the rapid development of resistance and the risk of C diff
