What is spironolactone
It is an aldosterone antagonists but it has Potassium sparring effects
MOD Potassium sparring diuretics
relatively weak diuretics alone
in combination with other diuretics they can counteract potassium loss
acts on distal convulted tubules - inhibits reasborption of sodium by epithelial sodium channels
SE Potassium sparring diuretics
uncommon at low doses
GI upset
with other diuretics - dizzy, hypotension, urinary Sx
risk of hypo/hyperkalaemia or hyponatreamia should not be ignored
Warnings of Potassium sparring diuretics
Avoid in severe renal
hyperkalaemia
hypokalaemia - should not be start with it
like other diuretics avoid in state of volume depletion
Interactions of Potassium sparring diuretics
dont use with other potassium elevating drugs - potassium supplements and aldosterone antagonists
digoxin and lithium renal clearance may be altered
Why monitor U&Es when adding Spironolactone in step4 HTN treatment
Because of risk of hyperkalaemia and hyponatreamia since potassium retention is increased with Spironolactone but sodium and water is excreted
