pathophysiology?
A
inflammation & fibrosis of intrahepatic AND/OR extrahepatic bile ducts –> obstruction to flow of bile
back pressure ultimately leads to hepatitis, fibrosis and cirrhosis

- men
- 3rd-5th decade
- associated w/ UC
- FH

symptoms?
A
- fatigue
- RUQ abdo pain
- itching
- jaundice

signs?
A
signs of cirrhosis and liver failure

Ix?
A
- LFT- ALP most deranged
- Autoantibodies
- non sensitive and specific to PSC but main ones are p-anca, ANA & anticardiolipin antibodies
- gold standard- MRCP

Mx?
A
- ursodeoxycholic acid
- colestyramine
- liver transplant
- ERCP- to dilate & stent any strictures
monitor for complications

Complications?
A
- cirrhosis and liver failure
- cholangiocarcinoma
- colorectal cancer
- acute bacterial cholangitis
- biliary strictures
- fat soluble vitamin deficiencies
