Aetiology?

Auto-immune

• auto-abs towards gastric parietal cells & intrinsic factor –> pt develops pernicious anaemia

Bacterial- H.Pylori

Chemical- NSAIDs, alcohol, bile reflux

• dyspepsia
• nausea, vomiting and loss of appetitie

Ix?

Answer 3

• H.Pylori urea breath test
• H.Pylori faecal antigen test
• FBC

to consider:

• endoscopy
• gastric mucosal histology
• serum vit B12

Question 4

Mx?

Answer 4

depends on aetiology

• H.Pylori–> eradication therapy
• NSAIDs / alcohol –> reduce / discontinue use
• auto-immune –> cyanocobalamin (vit b12)

+ symptomatic mx: H2 antagonists / PPI

Question 5

complications?

Answer 5

• progression to peptic ulcer disease
• gastric carcinoma / lymphoma
