- gallstone impaction at terminal ileum
- stone causes pressure necrosis of gall bladder wall & goes into small intestine

A
AXR–> SBO, air in biliary tree, stone might be seen

A
enterotomy + stone removal

A
cessation of peristalsis due to failure of neuromuscular mechansim of intestine

A
as per bowel obstruction

A
- post - op following major abdominal surgery
- infective - peritonitis
- trauma
- electrolyte imbalance

clinical picture of paralytic ileus?
A
- no pain as no peristalsis
- dead silent abdo
- false shifting dullness

A
- AXR
- dilated small loops of bowel
- CT
- dilated small bowel loops w no transition point

Mx for paralytic ileus?
A
- NG tube
- restoration of fluid & electrolyte balance
- treat underlying cause
eventually bowels should regain function
