What are they used for?
reopen occluded arteries in MI or stroke - asap - improve mortality
What is the fibrinolytic cascade?
plasminogen to plasmin which breaks up fibrin leading to clot lysis
What is the side effect of alteplase and duteplase?
can cause haemorrhage - controlled by oral tranexamic acid - inhibits plasminogen activation
How are alteplase and duteplase administered?
IV infusion
How is streptokinase administered?
PCI (percutaneous coronary intervention) or IV
Why are the side effects of streptokinase?
may cause an allergic reaction - don’t give if recent streptococcal infection
What are alteplase and duteplase?
recombinant tissue plasminogen activator - selective for clots - more effective on fibrin bound plasminogen than plasma plasminogen
Why can’t streptokinase be administered after 4 days?
action is blocked - generation of antibodies
