What is sacunitril-valsartan used for?
replaces ACE inhibitor in moderate/severe CCF - leave 48hrs gap when switching
What is sacunitril-valsartan?
combination of ARB and neprilysin inhibitor
What is Ivabradine used for?
still tachycardic despite optimal use of beat blockers or if beta blockers are contra indicated
When is spironolactone used?
moderate/severe CCF
What other drugs are used in CCF?
ACE inhibitors/ARBs, nitrates
What are the initial risks of beta-blockers?
hypotension, worsening dyspnoea
Give an example of a loop diuretic
furosemide
What is the mechanism of action of a neprilysin inhibitor?
blocks natriuretic peptide breakdown - boosts natriuretic peptide levels
What is the dose of beta blockers in heart failure?
start low and increase slow - good in long term - can worsen CCF in short term
What is cardiac resynchronisation therapy?
3 pacemakers inserted - force left and right ventricles to contract together
What diuretics are used in mild CCF?
thiazide diuretics
What is the effect of Ivabradine?
slows HR in sinus rhythm
What is digoxin used for?
CCF with atrial fibrillation
What are the side effects of spironolactone?
hyperkalaemia, renal dysfunction, gynaecomastia
What is cardiac resynchronisation therapy used for?
prolonged QRS
