minor features of rheumatic fever
fever, arthralgias, leukocytosis, increased ESR, prolonged PR interval
diagnosis for rheumatic fever
jones criteria; requires two major or one major and two minor
serologic markers in rheumatic fever
antistreptolysin O; anti-Dnase and anti-hyaluronidase antibodies
management of rheumatic fever
eradication of strep infx; control of inflamm with NSAIDs only after dx been made; steroids for patients w severe heart involvement; longterm antimicrob prophylaxis
prognosis of rheumatic fever
no chronic sequelae of joint, skin, CNS manifestations; severe heart valve dysfunction/insuff
lyme disease
caused by burrelia burgdorferi that goes into blood stream w tick bite; tick must be attached for 36-48 hours before risk of transmission becomes substantial
early localized lyme disease
target rash; constitutional symptoms
early disseminated disease
within 1-4 months after the tick bite; cutaneous findings plus skin, constitutional, neuro, cardiac findings
late disease
5-12 months after transmission; occurs in 7% of kids; hallmark is arthritis
lab dx of lyme disease
two step procedure: ELISA then western blot for antibodies to B burgdorferi
