What is rape trauma syndrome?
a cluster of symptoms which occur during, immediately
following, and for a time period after a sexual assault
What 2 categories does the rape trauma syndrome fall under?
- Disorganization (the immediate acute phase), and 2. Reorganization (the
long term phase). The victim’s life may be affected physically, emotionally, sexually
and socially
During disorganization phase of rape trauma syndrome, what is the immediate impact?
Emotions expressed by the victim may range from
controlled (calm, composed, subdued) to expressive (angry, fearful, restless,
tense, crying, nervous)
During disorganization phase of rape trauma syndrome, what are the emotional reactions?
Fear, stress reactions to a perceived threat, fear
of being physically injured, mutilated or killed, humiliation, guilt, shame,
embarrassment, self-blame, anger, and helplessness. Victims may experience
mood swings, fluctuating emotions, temper tantrums, jumpiness, irritability, or
extreme caution
During disorganization phase of rape trauma syndrome, what are the physical reactions?
Decrease in appetite, sleep disturbances, nausea,
general feelings of body soreness, or pain or ailments specific to the assault.
During disorganization phase of rape trauma syndrome, what are the social responses?
Reliving the assault over and over, flashbacks,
depression, or low self-esteem. They may become less communicative and
isolated from social settings.
During disorganization phase of rape trauma syndrome, what are the sexual reactions?
Feeling fearful of intimacy, an abhorrence for any
sexual relations, or a general dissatisfaction
During reorganization phase of rape trauma syndrome, what are the psychological responses?
Nightmares, phobias, paranoia, or compulsivity
During reorganization phase of rape trauma syndrome, what are the physical problems?
Victims may experience gynecological and/or menstrual
problems, backaches, urinary infections, migraines, and/or eating or sleeping
disturbances.
During reorganization phase of rape trauma syndrome, what are the social responses?
Victims may be seeking to change an aspect of their lives.
They may change residences, telephone numbers, move back in with parents, visit
family and friends, experience marital problems, or become overprotective of
children.
During reorganization phase of rape trauma syndrome, what are the sexual relationships like?
Sex may be something that is feared or not desired. Some
victims may have flashbacks during sexual relations while others may become
promiscuous.
What is consent?
Consent is a freely given yes. The person has the choice between
yes and no, and there are no bad consequences to either answer.
Consensual sex is the only legal sex
What is coercion?
Coercion is begging, lying, making someone feel guilty, arguing,
wearing someone down, trying to turn a “no” into a “yes,” or using
emotional blackmail
Is coercion legal?

What is the age of consent in PA?

What constitutes satutory sexual assault in PA?
Statutory Sexual Assault occurs when a person is 13, 14, or 15 and is
having sexual intercourse with someone who is FOUR or more years
older, and is not married to that person
What % of the time does a child know his/her abuser?
85%
What is the most common type of abuse?
Situational- no sexual preference for children, just occurs because the situation allows it
What are pedophiles?
Preferential sexual abusers, that are preferential for age and uses the grooming process
What is the grooming process?
identify vulnerable victim –> gain childs or parents trust —> desensitizes child to ouch (tickling) –> tells child to keep touching secret
In their lifetime, how many women are victim to sexual assault?
1 in 3
In their lifetime, how many men are victim to sexual assault?
1 in 6
What is the #1 date rape drug?
Alcohol
What is GHB?
Date rape drug, Sx begin withing 1/2 hour and lasts 4-6 hours
What is rohypnol?
10x as strong as valium, lose memory for 8-12 hours and can last 2-3 DAYS
What is ketamine?
Sx begin within 5-20 mins and last 2-8 hours
What are the 3 Sx of PTSD?
- re-experiencing the trauma (flashbacks). 2. emotional numbing 3.increased irratibility
What is the most important factor of recovery from rape trauma syndrome?
Immediate personal contacts after the assault
What are the 5 steps for the forensic rape examination?
Interview –> physical exam –> genital exam –> evidence recovery –> follow-up
What is the main goal of the rape interview?
to obtain a detailed and factual description of the entire sexual assault
What is the main goal of the physical rape exam?
To assess the condition of clothing, emotional state, injuries or pain
What occurs for 70% of rape traumas?
Posterior forchette tears
80% of injury in rape is from what?
penile penetration
What is the foley catheter technique?
A foley is inserted just past the hymen on pubertal girls –> baloon inflated with water –> slight traction on hymen to assess hymen
What is reflex anal dilation (RAD)?
internal anal sphincter dilates when the patient is put in the position in which the abuse routinely occurs
What specialized dye is used for the detection of motile sperm on the epidermis?
Toluidine blue
