Define sexual assault
attempted sexual touching of another person without his/ her consent and includes sexual intercourse (rape), sodomy (oral-genital or anal-genital contact), and fondling.
What is sexual abuse?
when a child engages in sexual activity, for which he/she cannot give consent, is unprepared for developmentally, cannot comprehend, and/or violates the law so social taboos of society
How many boys and girls are sexually abused during sometime in their childhood?
Worldwide 25% of girls and 9% of boys experience some form of sexual abuse during their childhood
What puts kids at risk for sexual abuse?
Increased risk is associated with poor parent-child relationships, poor relationships between parents, absence of a protective parent, and the presence of non-biologically related males in the house
Generally, what are the indicators of sexual abuse?
Most of the indicators of sexual abuse are nonspecific; the more specific ones include rectal/genital bleeding and STI’s that are not contracted perinatally
When taking a history for a child of sexual abuse, what should you include?
History should be comprehensive and include the child’s current and past medical problems, as well as social and family histories.
How can you gain more accuracy and information when taking a childs history for abuse?
Ask open ended questions, allow the pt to draw or use a doll, and speak in words that the child understands.
Which parts of the body must you examine for a rape victim?
mouth, breast, genitals, inner thighs, perineal region, buttocks and anus
What should the oral cavity be examined for?
forced entrance; bruising or petechiae
What should the female genitalia be examined for?
erythema, abrasions, lesions, or tears and if a discharge is present they should be checked for an STI
True or False: you never do a speculum exam on a prepubertal girl.
True (unless there is active bleeding with an unknown cause)
What changes to the hymen might indicate abuse?
notching, lacerations, or fusion to the labia minora.
What are the thing you will look for on the penis and scrotum of abuse patients?
erythema, bruising, acute or chronic trauma, bite marks, or abrasions
Generally, what are some differential diagnoses to sexual abuse?
Any form of genital trauma, infection or dermatological conditions, including genetic conditions that involve the perineum or anus.
