Class II Antidysrhythmic; Beta-blocker
Metoprolol Mechanism?
As a cardio-selective beta-blocker, it binds to and blocks B-1 sympathetic receptors which block catecholamines. Causes negative chronotropic, inotropic, and dromotropic effects.
Metoprolol Indications?
- Atrial fibrillation and atrial flutter with rapid ventricular response in patients taking b-blockers.
- SVT refractory to vagal maneuvers and adenosine
Metoprolol Contraindications?
- Hypotension
- Decompensated cardiogenic shock
- Suspected cocaine ingestion
- WPW Syndrome
- Bradycardia
- High Degree Heart Block
Metoprolol Side Effects/Precautions?
- Syncope
- Dizziness
- Wheezing
- Shortness of breath
- Nausea
- Weakness
- Bradycardia
- Heart block
- Hypotension
- CHF/pulmonary edema
Metoprolol Adult Dose?
5 mg slow IV/IO every 5 minutes
