Sympathomimetic
Epinephrine Mechanism?
Non-selectively binds to a1, b1, and b2 adrenergic receptors to increase inotropy, chronotropy, dromotropy, automaticity, peripheral vasoconstriction, and bronchodilation.
Epinephrine Indications?
-Cardiac arrest
-Anaphylaxis
-Severe bronchospasm
-Symptomatic bradycardia
-Non-hypovolemic shock
Epinephrine Contraindications?
- None in cardiac arrest
- Hypovolemia
- Tachydysrhythmia
- Relative: heart disease, tachycardia, hypertension
Epinephrine Side Effects/Precautions?
- Headache
- Syncope
- Nausea/vomiting
- Tachycardia
- Hypertension
- Dysrhythmia
- Increased myocardial oxygen demand
Epinephrine Adult Dose?
- Cardiac arrest: 1 mg 1:10,000 IV/IO every 3-5 minutes
- Anaphylaxis/severe bronchospasm: 0.3-0.5 mg 1:1,000 IM
- Shock/bradycardia: 2-10 mcg/min IV/IO infusion
Epinephrine Pediatric Dose?
- Cardiac arrest: 0.01 mg/kg (0.1ml/kg) 1:10,000 IV/IO every 3-5 minutes
- Anaphylaxis/severe bronchospasm: 0.01 mg/kg 1:1,000 IM
- Shock/bradycardia: 0.1-1.0 mcg/kg/min IV/IO infusion
Epinephrine Onset of Action?
Immediate
Epinephrine Duration of Action?
5-10 minutes IV, 20-30 minutes IM
Epinephrine Max Dose Adult/Pediatric?
Adults have no maximum. Pediatric is 0.3 mg/kg for anaphylaxis and symptomatic bradycardia.
