Glucagon Mechanism?
Liver - causes activation of cAMP, causes a cascade of effects ultimatley allows glucose to be released from glycogen polymers
Heart - cAMP increase causes calcium entry with a slight positive chronotropy, inotropy, and dromotropy
Glucagon Indications?
- Hypoglycemia where IV access cannot be obtained
- Beta-blocker overdose
- Calcium channel blocker overdose
Glucagon Contraindications?
- Hypersensitivity
- Hyperglycemia
Glucagon Side Effects/Precautions?
- Dizziness
- Headache
- Tachycardia
- Hypertension
- Nausea/vomitng
Glucagon Adult Dose?
- Hypoglycemia: 0.5-1.0 mg IM, repeat in 5-10 minutes
- Beta-blocker, calcium channel blocker toxicity: 3-10 mg IV/IO push followed by 3-5 mg/hr infusion.
Glucagon Pediatric Dose?
- Hypoglycemia: 0.05-0.1 mg/kg IM, maximum 1 mg
- Beta-blocker, calcium channel blocker toxicity: 0.03-0.15 mg/kg IV/IO push followed by 0.03-0.10 mg/kg/hr infusion
Glucagon Onset of Action?
1 minute
Glucagon Duration of Action?
60 - 90 minutes
Glucagon Max Dose Adult/Pediatric?
Adult: Variable, dependant on theraputic affect. Peds: 1mg
Glucagon Routes of Administration?
IV/IO/IM
