Class IV Antidysrhythmic; Calcium Channel Blocker
Diltiazem Mechanism?
Inhibits extracellular calcium ion influx across membranes of myocardial cells and vascular smooth muscle cells with substantial inhibitory effects on cardiac conduction system, acting principally at AV node, with some effects at sinus node
Diltiazem Indications?
- Atrial fibrillation and atrial flutter with rapid ventricular response
- SVT refractory to adenosine
Diltiazem Contraindications?
- Heart Blocks
- Hypotension
- Decompensated Cardiogenic Shock
- Ventricular Tachycardia
- WPW Syndrome
Diltiazem Side Effects/Precautions?
- Syncope
- Nausea/Vomiting
- AV blocks
- Bradycardia
- Hypotension
- Chest pain
- VF/VT
- CHF
Diltiazem Adult Dose?
- 10mg IV/IO over 2 min repeat every 10 min.
- Maintenance infusion: 5-15 mg/hr titrated to heart rate
