Class V Anti-Dysrhythmic, Mineral, electrolyte
Magnesium Sulfate Mechanism?
Competes with with calcium at NMJ to affect distribution of calcium uptake and release. Decreases amount of Ach released at motor end plate and slows SA node impulses and conduction time
Magnesium Sulfate Indications?
- Severe bronchospasm associated with asthma and COPD
- Torsades de Pointes
- Eclamptic seizures
- Hypomagnesemia
- VF/VT arrest refractory to amiodarone
- Dysrhythmia due to digitalis toxicity
Magnesium Sulfate Contraindications?
- Heart block
- Persistent hypertension
- Dialysis patients
- Hypocalcemia
Magnesium Sulfate Side Effects/Precautions?
- NS depression
- Respiratory depression
- Paralysis
- Bradycardia
- Burning at injection site
- Decreased reflexes
- Nausea/vomiting
- Hypotension
- Vasodilation
- Itching/rash
Magnesium Sulfate Adult Dose?
Adult:
Cardiac: 1 g over 1 minute IV/IO
Respiratory: 2 g in 100 ml of NS over 10 minutes IV/IO
Eclampsia: 4g in 100 ml over 5-10 minutes IV/IO
Peds: 25-50 mg/kg in 100 ml over 10 minutes
Max-Adults(Peds): 10g/day(2g/day)
Magnesium Sulfate Pediatric Dose?
25-50 mg/kg in 100 ml over 10 minutes; max 2 g
