Amiodarone Classification?
Class III Antidysrhythmic
Amiodarone Mechanism?
Blocks potassium channels causing prolongation of the action potential duration in all cardiac tissues. Also has blocking action on sodium channels, calcium channels and “alpha” and ‘beta” adrenergic receptors.
Amiodarone Indications?
- Cardiac Arrest (VF, Pulseless VT)
- Stable Wide Complex Tachycardia
Amiodarone Contraindications?
- Bradycardia
- High degree Heart block
- Hypersensitivity
- Hypotension (except in Cardiac Arrest)
- Cardiogenic shock
Amiodarone Side Effects/Precautions?
- Dizziness
- Cough
- Shortness of breath
- CHF/Pulmonary edema
- Nausea
- Prolonged QT Interval
- Bradycardia
- Heart block
- Hypotension
- Burning at IV site
Amiodarone Adult Dose?
- VF/VT arrest: 300 mg IVP followed by 150 mg IV/IO in 3-5 minutes
- Perfusing VT/malignant PVCs: 150 mg in 100 ml NS or D5W over 10 minutes IV drip
- Maintenance infusion: 1 mg/min for 8 hours, then 0.5 mg/min
- Do not exceed 2.2 grams per day
Amiodarone Pediatric Dose?
- VF/VT arrest: 5 mg/kg (max 300 mg) IV/IO every 3-5 minutes, maximum 15 mg/kg or 450 mg
- Perfusing VT/malignant PVCs: 5 mg/kg IV/IO over 20-60 minutes

Immediate
Amiodarone Duration of Action?
30-45 Minutes
Amiodarone Max Dose Adult/Pediatric?
Adult: NTE 2.2 grams in 24 hours
Peds: 450mg
