Proximal: Ilium posterior to posterior gluteal line; dorsal surface of sacrum and coccyx ; sacrotuberous ligament

Distal: most fibers end in the iliotibial tract (IT Band), (which inserts into lateral condyle of tibia), some fibers insert on gluteal tuberosity
