GH instability Flashcards
MOI for anterior GH dislocation
TUBS: trauma, unidirectional, bankart, surgery
Abduction and ext rotation
FOOSH
forcefull throwing
What is a Bankart lesion (and reverse Bankart lesion)
Anterior inferior labral tear that occurs with an ant. GH dislocation
Reverse occurs posterior and inferiorly on the labrum from a post. GH dislocation
What is a Hill Sachs lesions (and reverse Hill Sachs lesion)
Impaction type fx on the posterior humeral head from an ant. dislocation
Reverse occurs on the anterior humeral head with a post. dislocation
What nerve do you need to assess with a dislocation
Axillary N.
The lateral part of the deltoid loses sensation and can’t abduct
What might occur with an ant. GH dislocation in adults
Evaluate for a rotator cuff tear, especially the subscapularis
How long do you have to fix a subscapularis tear
4 weeks
Tx of ant. dislocation
reduction before spasm
PT
sling for about a week
methods for reducing dislocations
Simson’s (oldest)
Traction-countertraction technique (most common)
forward elecation maneuver
scapular manipulation technique
A 12 year old pt. has shoulder pain. Could it be a frozen shoulder?
No more like a fx/dislocation
What is important to not miss on a physical exam for an ant. dislocation
Axillary nerve palsy and large rotator cuff especially subscapularis
MOI for post. GH dislocation
rarely occur
Force along arm while in IR and slight adduction
Seizure
Electrical shock
Evaluation of posterior dislocation
internally rotated arm
prominence of the humeral head on the post. shoulder
coracoid may look more prominent
GH multidirectional instability MOI
AMBRI: atraumatic, multidirectional, bilateral laxity, rehab, inferior capsular shift
What is a SLAP lesion and four types
Superior labrum anterior to posterior 1-fraying of labrum 2-labrum tears off of glenoid 3-bucket handle tearing 4-tears into the biceps tendon
SLAP repair
don’t often need to be repaired but many are
