Symptoms of Extrapyramidal syndrome:

Sign:

A

Negative sign – bradykinesia, akinesia and abnormal posture adjustment

  • Positive sign* – dyskinesia, hyper-tonicity and rigidity
  • Basal ganglion* which is associated with initiation and adjustment of voluntary movement by enhancing the agonist muscle signals whereas diminishing the antagonist muscle signals from the premotor gyrus.

After sending information to pre-central motor cortex, signals transmit to the lower motor neuron by pyramidal tract

Parkinson disease

  1. Degeneration and depigmentation of neurons in substntia nigra.
  2. Depletion of dopamine in caudate nucleus and putamen
  3. Bradykinesia and hypokinesia (difficulty in initiating and performing volitional movement)
  4. Rigidity (small steps, bend forward)
  5. Resting tremor
  6. Finger tapping test, balance push back test
  7. MRI, EEG
  8. Should be differentiate from essential tremor
