Exam 4: Perineum Flashcards
Perineum
A diamond-shaped space that is part of trunk below the pelvic diaphragm. Divided into 2 triangles by a line connecting 2 ischial tuberosities.
Triangles of perineum
Urogenital triangle
Anal triangle
Urogenital Triangle position
Lies in horizontal plane
Anal Triangle position
Tilted upward
Urogenital triangle boundaries
- Anterior: pubic symphysis
- Anterolateral: ischiopubic rami
- Posterior: Transverse line through ischial tuberosities
Layers of urogenital triangle
- Skin
- Superficial Perineal Pouch
- Perineal Membrane
- Deep Perineal Pouch
- Inferior Fascia of the Pelvic Diaphragm
Superficial Perineal Pouch made up of
- Superficial perineal fascia
2. Deep perineal Fascia
Superficial Perineal Fascia
Fatty Layer (superficial) Membranous layer (deep)
Colles’ Fascia
Membranous layer of superficial perineal pouch. Continuous with Dartos layer of the scrotum and Scarpa’s fascia
Superficial Perineal Pouch contents
- Perineal body
- Superficial transverse perineal muscle
- Ischiocavernosus muscle
- Crura of the Penis (Clitoris)
- Bulbospongiosus Muscle
- Bulb of the Penis (Vestible)
- Greater Vestibular Gland (females)
- Branches of the internal pudendal artery
- Branches of the Pudendal Nerve
Perineal body
Fibromuscular mass located between anal canal and bulb of penis
Muscle attachments on Perineal body
- Superficial transverse perineal
- Deep Transverse Perineal
- Bulbospongiosus
- Levator Ani
- External Anal sphincter
Episiotomy
Incision from posterior vaginal wall to perineal body. Prevents uncontrolled tearing of perineum
Muscle that does NOT attach to perineal body
Ischiocavernosus
Superficial Transverse Perineal Muscle Nerve
Perineal branch of pudendal nerve
Ischiocavernosus Nerve
Perineal branch of pudendal nerve
Bulbospongiosus muscle (Male)
Perineal branch of pudendal nerve
Impedes venous return by compressing the bulb of the penis, maintaining the erection. Constricts corpus spongiosum to empty the urethra at the end of micturition and during ejaculation
Bulbospingosus (Female)
Pudendal Nerve
Constricts vaginal orifice and maintains erection of clitoris
Greater Vestibular Gland
2 ovoid glands which are located in the superficial perineal pouch, posterior to the bulb of the vestibule
Greater Vestibular Gland homologous to:
Bulbourethral gland in the male.
Secretes mucus which lubricates the vagina during sexual intercourse
Mechanism of Erection
- Parasympathetic activation (S2,3,4)
- Flow of blood compresses venous plexuses
- Bulbospongiosus and ischiocavernosus contract
Deep Perineal Pouch contents
- Deep transverse perineal muscle
- External urethral sphincter
- Membranous part of urethra
- Bulbourethral gland (males)
- Branches of internal pudendal artery
- Pudendal nerve
Deep transverse perineal muscle
Inserts of median raphe in males or walls of the vagina in females
Stabilizes the perineal body and supports the prostate gland or vagina
External urethral sphincter Muscle
May be damaged during parturition and can lead to incontinence
External Urethral Sphincter muscle (males)
Inferior Part
Superior part
Inferior Part of external urethral sphincter muscle (male)
True anatomical sphincter which encircles the urethra below the prostate
Superior part of external urethral sphincter muscle (male)
Trough-like portion within the isthmus of the prostate
External urethral sphincter muscle (female)
Sphincter urethrovaginalis
Compressor urethrae
Superior part
Sphincter urethrovaginalis
Inferior part of External urethral sphincter muscle that encircles the urethra and vagina
Compressor Urethrae
Forms a band anterior to the urethra
Superior part of External urethral sphincter muscle (female)
Trough-like portion
Function of External urethral sphincter muscle
Compresses urethra at the end of micturition
Bulbourethral glands
2 glands that lie in the deep perineal pouch
Bulbourethral glands homologous to:
Greater vestibular glands of the female
Secrete a mucus-like substance (minor part of semen)
Bulbourethral gland empties into:
Spongy urethra
Anal triangle Boundaries
Anterior: transverse line trough ischial tuberosities
Posterolateral: sacrotuberous ligament
Posterior: tip of the Cocccyx
Contents of anal triangle
Anal canal, external anal sphincter, ischioanal fossa
Ischioanal fossa Lateral wall
Formed by obturator fascia
Ischioanal Fossa contains
- Fat
- Pudendal canal
- Inferior rectal nerve and vessels
- Perineal branch of posterior cutaneous nerve of thigh
Pudendal canal
Formed by the obturator fascia and lies on the lateral wall of ischioanal fossa
Pudendal canal transmits
Internal pudendal vessels and pudendal nerve
Blood supply to perineum
Internal Pudendal artery
Internal Pudendal Artery
Arises from internal iliac artery. Travels in pudendal canal with the pudendal nerve
Branches of Internal Pudendal Artery
- Inferior rectal artery
- Perineal Artery
- Posterior scrotal (Labial) artery
- Artery of the bulb
- Urethral Artery
- Deep artery of the penis or clitoris
