What is nappy rash?
Contact dermatitis in the nappy area
What causes nappy rash?
Friction between skin and nappy
Contact with urine and faeces in a dirty nappy
When do babies typically get nappy rash?
9-12 months
Most babies get nappy rash at some point
What can nappy rash lead to?
Breakdown in skin and warm moist environment in nappy can lead to candida infection or bacteria
What bacterial organism usually causes an infection in nappy rash?
Staphylococcus
Streptococcus
What are the risk factors for nappy rash?
Delayed nappy changing
Irritant soaps and vigorous cleaning
Poorly absorbent nappies
Diarrhoea
Oral antibiotics - predispose to candida
Pre-term infants
How does nappy rash present?
Sore, red, inflamed skin in nappy area
Spares skin creases
Red papules beside affected skin
What can severe and prolonged nappy rash lead to?
Erosions
Ulceration
How can you distinguish between nappy rash and candida infection?
Candida infection
- Rash extends into skin folds
- Large red macules
- Well demarcated scaly border
- Circular pattern to rash spreading outwards
- Satellite lesions
Can also have oral thrush
How is nappy rash managed?
Switch to highly absorbent nappies
Change nappy and clean skin as soon as possible
Use water or gentle alcohol free products
Ensure nappy area is dry before replacing
Maximise time not wearing nappy
If candida or bacterial infection, give anti-fungal cram or antibiotic
What are the complications of nappy rash?
Candida infection
Cellulitis
Jacquet’s erosive diaper dermatitis
Perianal pseudoverrucous papules and nodules
