What does the enzyme make a lot of?
Enzymes
What does ALT do?
Takes an amine group away from alanine
The amine group becomes urea
The rest of the molecule feeds into gluconeogenesis
When is ALT in the liver turned on?
When you need to make glucose
Is ALT normally found in blood?
No, it is only in the blood if it leaks out of the liver
What is included in LFTs?
AST ALT GGT Alk phosph Bilirubin Albumin Fibrinogen Clotting factors Prothrombin time APTT
What will happen to albumin and fibrinogen in liver failure and why?
They will fall as the liver usually makes them but if its failing they might make less
If alk phosph is very high what is the likely type of jaundice?
Obstructive
If someone presents to A&E with painless jaundice what are you worried about?
Cancer- always think about cancer if someone presents with painless severe jaundice
If allk phopsh, alt and asp are all slightly raised with mild jaundice what is the likely diagnosis?
Hepatitis
If urine is positive for urobiliogen when type of jaundice does this indicate?
Hepatitis
If urine is negative for urobilinogen what jaundice does this indicate?
Obstructive
What signs and symptoms are seen in obstructive jaundice?
Itchy skin, might see scratch marks
Pales stool
Does normal urine contain urobiliniogen?
Yes
Why is there no itching in hepatitis?
The liver doesn’t make bile salts
