Heterotrophic ossification
idiopathic development of bone in soft tissue
sudden loss of ROM, local edema, erythema and warmth, nonseptic fever
tx: avoid vigorous stretching, maintain ROM, optimize w/c positioning, requires surgical resection

age of onset: >40 years
progression: slowly over many years
manifestations: cartilage degradation, altered joint architecture, osteophyte formation
joint involvement: affects a few, usually asymmetrically
-DIP, PIP, 1st CMC of hands
- cervical and lumbar spine
- hips, knees, 1st MTP of feet
joint signs and sxs: morning stiffness (<30mins), increased jt pain with weight bearing and strenuous activity; crepitus and decreased ROM

progression: suddenly within weeks or months
manifestations: inflammatory synovitis and irreversible structural damage to cartilage and bone
joint involvement: many joints, usually bilateral
-MCP and PIP of hands, wrists, elbow, shoulders
- cervical spine
- MTP, talonavicular, ankle
joint signs and sxs: redness, warmth, swelling, prolonged morning stiffness, increased jt pain with activity
systemic signs and sxs: sickness, fatigue, rheumatoid nodules, ocular, resp, hematological and cardiac sxs
