COMPOUND SODIUM LACTATE (HARTMANN’S SOLUTION): INDICATIONS
A
- Provide sodium and water through IV
- Expand circulating volume in states of circulatory failure (eg shock) - done as a fluid challenge

COMPOUND SODIUM LACTATE (HARTMANN’S SOLUTION): MOA
A
- Balanced salt solution, with its constituents designed to mimic serum
- In presence of adequate liver fxn - lactate metabolised to PYRUVATE, then to either glucose or carbon dioxide and water
- Release of bicarb in both cases
- Sodium = used to provide sodium and water intravenously and also for the expansion of circulating volume

COMPOUND SODIUM LACTATE (HARTMANN’S SOLUTION): ADVERSE EFFECTS
A
- Excessive administration can cause a fall in CO and severe heart failure by increasing left ventricular filling beyond the point of maximal contractility on the Starling Curve
- Oedema, relevant in patients who have received multiple fluid challenge, since about 80% of the administered volume will be lost into tissue fluid
- NaCL advantage - lower chloride content and less likely to cause hypercholaremic acidosis
