Why in News?

A

Recently, the Union Cabinet has approved the countrywide implementation of the Ayushman Bharat Digital Mission (ABDM) with the budgetary allocation of Rs 1,600 crore for five years.

Under the mission, citizens will be able to create their Ayushman Bharat health account numbers, to which their digital health records can be linked.
Ayushman Bharat is a flagship scheme of India which was launched as recommended by the National Health Policy 2017, to achieve the vision of Universal Health Coverage (UHC).

What is Ayushman Bharat Digital Mission?

A

It was launched in September 2021 by the Prime Minister through a video conference.
It aims to provide digital health IDs for all Indian citizens to help hospitals, insurance firms, and citizens access health records electronically when required.
The pilot project of the Mission had been announced by the Prime Minister from the ramparts of the Red Fort on 15th August 2020.
The project is being implemented in the pilot phase in six States & Union Territories.
The National Health Authority (NHA) under the Ministry of Health and Family Welfare will be the implementing Agency.

National Health Authority

A

The National Health Authority or the NHA is responsible for implementing India’s flagship public health insurance/assurance scheme Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (AB-PMJAY). NHA has been set-up to implement the PM-JAY at the national level. In the States, SHAs or State Health Agencies in the form of a society/trust have been set up with full operational autonomy over the implementation of this scheme including extending the coverage to non SECC beneficiaries.

National Health Authority is the successor of National Health Agency, which was functioning as a registered society since 23 May 2018. Pursuant to Cabinet decision for full functional autonomy, National Health Agency was reconstituted as the National Health Authority on 2 January 2019

The NHA is governed by a Governing Board chaired by the Union Minister for Health and Family Welfare, Government of India; and is headed by a full-time Chief Executive Officer (CEO) supported by Deputy CEO and Executive Directors. The CEO of the NHA functions as the Member Secretary of the Governing Board.

The Governing Board comprises Chairperson and 11 members.

Succeeding Indu Bhushan on 1 February 2021, Former TRAI chairman Ram Sevak Sharma is the current CEO of the NHA.

Functions of NHA

A

From 2018-19, the organization was under the control of the Ministry of Health and Family Welfare.[4] After 2019, the organization became independent and answerable to its own board of experts and policy makers.[4] The organization’s primary activity is to manage the Pradhan Mantri Jan Arogya Yojana (PM-JAY), which is a national health insurance program in India.[4] Other goals include improving access to health information and data for the public sector and supporting the Insurance Regulatory and Development Authority.[4]

In October 2019, the organization announced a collaboration with Google. In the collaboration, Google will provide digital training to staff and partners, while the organization will seek to increase public access to data.

What are the Features of the Mission?

A

Health ID:
It will be issued for every citizen that will also work as their health account. This health account will contain details of every test, every disease, the doctors visited, the medicines taken and the diagnosis.
Health ID is free of cost, voluntary. It will help in doing analysis of health data and lead to better planning, budgeting and implementation for health programs.

Healthcare Facilities & Professionals’ Registry:
The other major component of the programme is creating a Healthcare Professionals’ Registry (HPR) and Healthcare Facilities Registry (HFR), allowing easy electronic access to medical professionals and health infrastructure.
The HPR will be a comprehensive repository of all healthcare professionals involved in delivering healthcare services across both modern and traditional systems of medicine.
The HFR database will have records of all the country’s health facilities.
Ayushman Bharat Digital Mission Sandbox:
The Sandbox, created as a part of the mission, will act as a framework for technology and product testing that will help organisations, including private players intending to be a part of the national digital health ecosystem become a Health Information Provider or Health Information User or efficiently link with building blocks of Ayushman Bharat Digital Mission.

What are the Benefits and Related Concerns?

A

Expected Benefits:
Ensure ease of doing business for doctors and hospitals and healthcare service providers.
Enable access and exchange of longitudinal health records of citizens with their consent.
Create integration within the digital health ecosystem, similar to the role played by the Unified Payments Interface (UPI) in revolutionising payments.
The mission will improve “equitable access” to quality healthcare as it’ll encourage the use of technologies such as telemedicine, and enable national portability of health services.
Concerns:
The lack of a data protection bill could lead to the misuse of data by private firms and bad actors.
Exclusion of citizens and denied healthcare due to faults in the system are also a cause of concern.

Way Forward

A

The National Digital Health Mission (NDHM) still does not recognize Health as a justiciable right. There should be a push draft at making health a right, as prescribed in the draft National Health Policy, 2015.
In addition, the failure of a similar National Health Service (NHS) in the United Kingdom must be learnt from and the technical and implementation-related deficiencies must be proactively addressed prior to launching the mission on a pan India scale.
The standardisation of NDHM architecture across the country will need to find ways to accommodate state-specific rules. It also needs to be in sync with government schemes like Ayushman Bharat Yojana and other IT-enabled schemes like Reproductive Child Health Care and NIKSHAY etc.

What is eSanjeevani?

A

eSanjeevani is the first-ever online OPD (outpatient) consultation service offered by the government of India to citizens. According to the government, this is the first time the government of a country is offering a service of this kind to its citizens.

The scheme was started in November 2019.
It is run by the Ministry of Health and Family Welfare.
Also called the National Teleconsultation Service, it aims to provide healthcare services to patients in their homes.
It includes a structured and safe teleconsultation between a doctor and a patient through online mode (eSanjeevani OPD).
The eSanjeevani OPD portal and system has been developed by the Centre for Development of Advanced Computing (C-DAC) in Mohali.
The panel of doctors on the service is drawn by the state governments.
A few states like Jharkhand, Kerala, Punjab and Tamil Nadu, etc. have started offering specialised doctor consultation services also.
The service is also available on mobile application.

Difference between eSanjeevani and eSanjeevani OPD

A

eSanjeevani: It is a doctor to doctor telemedicine system that has been deployed nationally for the Ministry of Health & Family Welfare under Ayushman Bharat Scheme. It extends the reach of specialized healthcare services to masses in both rural areas and isolated communities. It tackles the issues of uneven distribution of healthcare personnel and infrastructure by bridging the divide that exists between urban and rural, rich and poor, in terms of access to healthcare services.

eSanjeevani was launched in 2019. The first state to launch this service was Andhra Pradesh.

eSanjeevani OPD: This is based on the eSanjeevani model and offers OPD consultation services to people in the comfort of their homes. This was launched during the COVID pandemic in April 2020. Here, doctor to patient teleconsultation is offered free of cost to the patients.

eSanjeevani OPD was launched in 2020.

PM Aatmanirbhar Swasth Bharat Yojana

A

PM Aatmanirbhar Swasth Bharat Yojana aims at developing the capacities of the primary, secondary, and tertiary health sector.
The Union Finance Minister Nirmala Sitharaman on February 1, 2021, announced a centrally sponsored scheme- Pradhan Mantri Aatmanirbhar Swasth Bharat Yojana under Union Budget 2021. The scheme is with an outlay of Rs. 64,180 crores over the period of six years.

The Budget 2021 was the first to be presented in the Parliament, after the coronavirus outbreak. With major investments to strengthen healthcare, it saw an emphasis on the healthcare infrastructure along with research and on the establishment of hi-tech laboratories.

The Union Finance Minister before mentioning the scheme in the Parliament stated that the investment in health infrastructure has been increased in this budget substantially. She added that while taking a holistic approach to health, the government focuses on strengthening three areas- Preventive, Curative, and well-being.

Objective

A

PM Aatmanirbhar Swasth Bharat Yojana aims at developing the capacities of the primary, secondary, and tertiary health sector.

The scheme will focus on strengthening the existing national institutions as well as create new institutions for detecting and curing new and emerging diseases. It will be an addition to NHM- National Health Mission.
