Cause of acute pancreatitis
Alcohol or gallstones
Symptoms of acute pancreatitis
Severe epigastric pain which may radiate to the back, vomiting common
Examination of acute pancreatitis
Epigastric tenderness, ileus and low grade fever. Cullen’s sign, Grey-Turner’s sign
Diagnosis of acute pancreatitis
Serum amylase, serum lipase and imaging (but not necessary for diagnosis)
Factors indicating severe pancreatitis
Age >55, hypocalcaemia, hyperglycaemia, hypoxia, neutrophilia, elevated LDH and AST
Management of acute pancreatitis
Fluid resus, analgesia, nil-by-mouth, or enteral nutrition.
Indications for abx
Infected pancreatic necrosis
Surgical option for gallstone cause of pancreatitis
Cholecystectomy
Surgical option for obstructed biliary system due to stones
ERCP
