Week 2 AntiFungal Drugs- Purnuske Flashcards
1
Q
Amphotericin B, Nystatin
A
Binds ergosterol creating holes in membrane
Kills yeast and mold
Resistance is rare
Binds to cholesteral and causes NEPHROTOXICITY
*can cross blood brain barrier
2
Q
“Azoles”
Fluconazole
Itraconazole
Ketoconazole
A
binds fungal p450 enzyme and prevents production of ergosterol.
Resistance- alter p450 enzyme
-may cause hepatotoxicity and nephrotoxicity
-AVOID WHEN PREGO
**Most widely used
3
Q
Terbinafine
A
blocks production of ergosterol by inhibits squalene epoxidase.
- kills dermatophytes
- resistance is rare
- drug interactions with Cyp 2D6
4
Q
Fluctosine (5-FC)
A
selectively taken up by fungi and converted to 5 fluorouracil in fungi. Interferes with DNA/RNA synthesis
- YEAST ONLY
- can lead to bone marrow suppresion
- TREAT W/ AMPHOTERICIN B
5
Q
Griseofulvin
A
- binds microtubules and inhibits sliding
- fungistatic for dermatophytes
- Resistance= change in B-tubulin, if patient isn’t complient may develop
- AVOID WHEN PREGO
- lipids increase oral absorption
6
Q
Caspofungin
A
cell wall inhibitor, blocks synthesis of glucan
- Fungicidal for candida
- Treats Candida albicans
- Resistance = change in glucan synthase gene
- may get fever or rash as site of injection
- Have to give through IV
