Who generally is required to notify the NTSB of a serious incident?
55.10 pg. 1 - The director of operations and/or the director of safety. If they are unavailable it should be done through the chief pilot.
When does the NTSB need to be immediately notified?

-An accident
-A flight control failure or malfunction
-Inability of a flight crew member to perform their duties as a result of injury or illness
-Failure of any internal turbine engine component that results in the escape of debris other than out the exhaust path
-Inflight fire
-Aircraft collision in flight
-Damage to property, other than aircraft, estimated to exceed $25,000 for repair or fair market value in the event of total loss, whichever is less

What should the crew due to prevent CVR erasure following an accident?
55.10 pg. 2 - Pull the CVR circuit breaker, and if possible make a logbook entry saying that the CVR circuit breaker has been deactivated.
How will dispatch let other aircraft know of a Frontier Airlines accident?
55.10 pg. 3 - They will send out a mass ACARS message advising all crews of which flight it was and where it crashed.
