urge
sudden and unstoppable urge to urinate… associated with neuropathy- diabetes, stroke, etc.
Stress
Urine leaks out during any form of exertion (exercise, cough, sneezing, laughing)
Mixed
urge and stress…
functional
no abnormality in the bladder, but the pt may be cognitively, socially, or physically impaired thus hindering access to the toilet
overflow
leakage that occurs when the quantity of urine stored in the bladder exceeds its capacity (often occurs without the urge to urinate, BPH is the most common cause)
long-acting anticholinergics
such as oxybutynin ER or transdermal (Oxytrol) often used initially
- less SE but can cause cognitive impairment
- Selective anticholinergic (darifenacin, solifenacin) less cognitive effects
- Generic (oxybutynin) have lower cost
Mirabegron (beta-3 agonist) has similar efficacy to anticholinergics w/ different SE GI upset, dizziness, headache and increased BP
