In this category, you will find skilled care facilities where patients can be provided with wound care, treatments, and ongoing monitoring as they regain health to be able to return home.Often patients who have been treated in a hospital will be transferred to a skilled care facility for further recovery.

Nursing homes provide help with daily life activities like bathing, dressing, and eating for clients who cannot do these things for themselves. Many nursing home residents are elderly, but some need the care because of disease or injury that makes them unable to care for themselves. (long term care)

What are community health centers

A

Community health centers receive public funding and provide services to the community at large. These can include public health clinics for at-risk populations, like drug users, homeless people, or those who cannot afford to pay for care. There are often services open to anyone in the community, such as services for victims of domestic violence, a hotline for people in crisis, prenatal care for pregnant women, and social services to help people access needed health care.
