How to use root casue analysis

A

Using root cause analysis, the first step is to identify what happened, how it happened, and why it happened through a systematic analysis of the event. The focus is on why the mistake happened, not who to blame, keeping in mind that most problems have more than one root cause. Once the causes are identified, actions and process changes that will prevent future errors can be identified. Once implemented, these new processes or systemic or physical changes should be a more permanent solution, rather than solving problems by constantly “putting out fires.”
