what is referral?
when people need healthcare they usually make an appointment with their GP, who then refers the patient to a specialist doctor, hospital or a medical staff at a clinic.
what are needs assessment?
people who require care will have a needs assessment which will be carried out by social services to see if the patient is eligible to the care that they require.
what do needs assessment see in a patient?
- their needs and how this impacts their care
- what matters to them the most in the care
- choices and goals which they have
- types of services, advice, facilities, information that will prevent or delay further needs from
- the needs of their family
- the limitation of cost and service provision
what is the eligibility criteria?
where local authorities use needs assessment to assess whether people meet the eligibility criteria for their care
what does the eligibility criteria consider in a persons needs?
- physical impairment or illness, whether they have it or at risk of it
- whether they are are unable to achieve two or more of the specified outcomes
- because of not achieving the specified outcomes, it affects their well being significantly
how many of the criterias does an adult need to meet to be eligible to care?
all three of the aspects

-maintain personal and home hygiene
-dress and go to the toilet appropriate
-care for other e.g. if they have a child

which people are entitled to specific needs?
- learning and physical
- mental ill health

how can individual preferences pose as a barrier to services?
- religious or cultural beliefs, they may reject some
- women who prefer to receive care from women
- terminally ill people who chose not to have treatment
- mentally ill people who cant make decisions for themselves
- an older person who wants to remain independent
