Tractional Retinal Detachment
caused by contraction of fibrous tissue (from NV) on retinal surface that pulls retina
no break or liquefied vitreous required
Causes of Tractional RD
*proliferative diabetic retinopathy
retinopathy of prematurity
proliferative vitreoretinopathy
Symptoms of Tractional RD
little since slowly progresses
no flashes/ floaters
VF defect
Signs of Tractional RD
detachment appears smooth and concave
initially shallow and does not go past ora
stationary
proliferative fibrous membrane
Tx of Tractional RD
vitrectomy
membrane dissection
Proliferative Vitreoretinopathy
cells migrate to surface of retina, form fibrous sheet- contracts causing rigid folds that can cause detachment
after RD repair
Exudative Retinal Detachment
separation of retina from RPE by accumulation of fluid
fluid from breakdown of blood retinal barrier
no break required
Causes of Exudative RD
Vascular- Coat’s disease
Inflammatory- post scleritis
Neoplastic- choroidal melanoma (NV)
Idiopathic
Symptoms of Exudative RD
fluctuating visual changes
Signs of Exudative RD
smooth convex/ dome shaped elevation of retina
subretinal fluid that can visibly shift
leopard spots from reabsorbed fluid
