Retinal Detachment
separation of photoreceptors from RPE
Retinal Break
full thickness defect (tear, hole, break) in sensory retina that can occur without RD
Rhegmatogenous RD
retinal break allows liquefied vitreous into space b/t NSR and RPE
needs a full thickness retinal break, traction to hold break open, liquified vitreous
Causes of Retinal Breaks
mechanical tearing
atrophic hole formation
PVD, trauma, traction
Vitreal Synchysis
normal aging process where vitreous liquefies
happens to everyone, normally w/o RD
Posterior Vitreous Detachment
PVD
normal aging process where vitreous contracts forward into post chamber
accompanied by synchysis (liquefying) and syneresis (shrinking)
Complications of PVD
normally none
can cause retinal tear around stronger attachment points; vitreous hemorrhage
Symptoms of PVD
flashes and floaters
Symptoms of RD
flashes/ floaters
curtain in vision
Signs associated with RD
APD if RD is extensive decreased IOP iritis pigment in ant vitreous- Shafer's sign retinal breaks
Appearance of Recent Rhegmatogenous RD
elevated retina with translucent appearance
wrinkled
moves with eye movements
Appearance of Old Rhegmatogenous RD
detached retina becomes thinned and atrophied
intraretinal cysts
demarcation lines
Tx of Rhegmatogenous RD
close retinal break laser scleral buckle vitrectomy retinopexy- gas bubble tamponade
Proliferative Vitreoretinopathy
PVR
membranes form on and under detached retina that contract to cause retinal folds
detached retina pulled together- funnel appearance
Causes of PVR
failure in RD surgery
diabetic traction RD with retinal breaks
trauma
