elongation of globe stretches the tissues of the eye
-6D or greater or >26mm
Signs of Degenerative Myopia
thinned retina- tigroid
focal chorioretinal atrophy
breaks in Bruch’s- lacquer cracks
peripapillary atrophy- myopic crescent
Complications of Degenerative Myopia
rhegmatogenous RD
CNV
foveal reinoschisis/ macular hole
Mx of Degenerative Myopia
correct refractive error- contacts better
anti-VEGF for CNV
vitrectomy for foveal schisis/ hole
surgery for RD
Angioid Streaks
breaks in abnormally brittle Bruch’s
wider and darker than lacquer cracks
Systemic Diseases associated with Angioid Streaks
PXE- Pseudoxanthoma elasticum
Paget disease
Hemoglobinopathies
Mx of Angioid Streaks
avoid contact sports; protective eyewear
communicate findings with PCP
Choroidal Folds
wrinkling of inner choroid, Bruch’s, and RPE
more likely in hyperopes
can be associated with sight or life threatening conditions
Choroidal Folds Presentation
parallel light and dark bands = thinner stretched and thicker compressed RPE
horizontal
Causes of Choroidal Folds
hyperopic papilledema tumors scleral buckles thyroid ophthalmopathy posterior scleritis
Hypotony Maculopathy
impaired macular function due to very low IOP
Causes of Hypotony Maculopathy
glaucoma filtering surgery penetrating trauma cyclodialysis RD inflammation
Signs of Hypotony Maculopathy
chorioretinal folds, often on macula
retinal folds
low IOP <5
Mx of Hypotony Maculopathy
seal leak
treat inflammation, repair tear
Solar Maculopathy
foveal damage caused by staring at the sun
