Where shall the accountability form be placed?
The glove compartment or other suitable location
When and by whom shall the accountability form be checked?
at 0800 by the company officer
For accountability purpose what shall companies self-deploying do?
notify IC of their location and intended function
How many levels of accountability are there?
3 Levels
Level I accountability shall be utilized at?
- initial operations at residential structure fires of 3 units or less
- Level I gas emergencies
- carbon monoxide investigations
- Incident or potential incidents that could involve IDLH
- mass casualties
note: it is the lowest level
Level II accountability shall be utilized at?
- Residential fires greater than 3 units
- commercial fires
- 2nd alarm or greater
- Level II gas emergencies
- Any time order by IC
note: heightened
When an increase of accountability is issued what must be done?
Fire alarm shall activate emergency warble tone, and all companies must reunite as soon as possible
Level III accountability shall be utilized at?
- Confined space
- trench rescue
- terrorist attack
- active shooter
- hazardous incident
- Ice rescue
- radiological incident
- when ordered by IC
What must be done with members entering and exiting hazardous are or hot zone?
They must be documented:
- name
- scba
- cylinder level
- time in & out
Whenever the off-duty platoon of any unit reports for duty at the beginning of a tour and finds that such unit has responded to an alarm of fire or other emergency, what should be done?
Co. Officer upon arrival of all members shall notify fire alarm
If a member who is substituting for another member and reports at a time other than shift change and the company has responded to an alarm of fire or other emergency, what should be done?
the member must contact fire alarm and what for IC to make a decision.
If he is told to respond to the scene he must first go to the IC
What are the ways IC can have roll call conducted?
- face to face
- full roll call via dep. radio
- limited roll call via dep. radio
- have fire alarm conduct full/limited roll call via. dep radio
