SERIOUS INCIDENT REPORT Flashcards
Serious Incident is defined as:
- Loss of life, limb, eyesight, or serious injury
- Loss of or substantial damage to government or civilian property greater than 2k
- Exigent circumstance in which any step beyond verbal commands in the RUF is used
- Any threat to CD personnel, the agencies they support, or facilities they occupy
- Initial reporting will be by telephone call or email. Written report will follow.
Counter drug accident is:
unplanned event or series of events, which results in one or mor of the following:
- Occupational illness to Cd personnel
- Injury to TFM on or off duty
- Damage to public or private property
- Damage to CD property over 5k
- Injury to non-Counter drug personnel and/or damage to equipment as a result of Counter drug operations
- Police report/encounters are required via spot report to CD J3
Class A accident (report in 24hrs):
total cost of reportable property damage is 1mil or more
Class B accident (report in 24hrs):
total cost greater than 200k-1mil
Class C accident (report in 3 working days):
10k-200k
Class D accident (report in 3 working days):
The total cost of total reportable property damage greater than 2k-10k
Class E aviation accident (report in 5 working days):
An Army aviation incident in which the resulting damage cost and injury severity do not meet the criterion for Class A-D
Class F aviation accident (report in 5 working days
An army foreign Object Damage (FOD) incident
