what is psoriatic arthritis?
- inflammatory arthritis associated with psoriasis
- a type of seronegative spondyloarthropathy
do all patients with psoriatic arthritis have psoriasis?
10-15% of patients with psoriatic arthritis do not have psoriasis
is psoriatic arthritis seronegative or seropositive?
-seronegative
what are the patterns of psoriatic arthritis?
- Symmetrical polyarthritis
- Asymmetrical pauciarthritis
- Spondylitic pattern
describe how a symmetrical polyarthritis pattern psoriatic arthritis may present?
- presents similar to RA
- more common in women
- affects hands, wrists, ankles and DIPs (MCP joint is unaffected unlike RA)
describe how asymmetrical pauciarthritis pattern psoriatic arthritis may present
- pauciarthritis describes how only a few joints are affected
- affects mainly the digits (fingers and toes) and the feet
describe how a spondylitic pattern of psoriatic arthritis may present?
- back stiffness
- sacroilitis
- atlanto axial involvement
what is the most severe form of psoriatic arthritis?
-arthritis mutilans
how does arthritis mutilans appear?
- this is the most severe form of psoriatic arthritis!
- causes osteolysis (destruction) of the bones of the fingers and toes which causes digit shortening giving an appearance that is often called ‘telescopic finger’
what is the treatment for psoriatic arthritis?
Similar to RA!
- DMARDs (usually methotrexate)
- Anti TNF available for those who do not respond to standard treatment
- joint replacement can be considered in larger joints which are severely affected
