Novel/future therapies Flashcards
What are the limitations of current RA therapies?
Only treat symptoms Disease is progressive Patients often don't respond Serious SEs with long term use Expensive
What do matrix metalloproteinases do?
Degrade bone and cartilage and promote angiogenesis.
Increase circulating TNF.
Name 2 immunosuppressants
Cyclosporin
Tacrolimus
Immunosuppressants have selective effects on what?
T lymphocytes
What do Type IV PDEIs do?
Increase cAMP and suppress TNF translation
What produces IL-17?
Th17
What does IL-17 do?
Synergises with and enhances effects of TNF
What can IL-17 upregulate?
NFkB, MAPK, IL-6, COX2 and NO
What do Regulatory T cells do?
Dampen down immune cell activation
What do regulator T cells secrete ?
Anti-inflammatory cytokines e.g. IL-10, TGF-B
B cells in RA patients show what?
increased B7-2 and decreased FcR
What does a-TNF therapy decrease?
B7-2 expression
Rituximab is given when?
In combination with methotrexate to patients failing to respond to other treatments including anti TNF
