A
Heavy menstrual bleeding

Menorrhagia referral
A
- intermenstrual bleeding
- postcoital (sexual intercourse) bleeding/pain
- age under 18 or older than 45
- OTC failure (3 cycles without improving)
- renal failure
- family uterus cancer

Menorrhagia referral part 2
A
- active VTE
- thrombophilia (tendency to form blood clots)
- pregnancy/ breastfeeding
- obesity
- diabetes
- taking tamoxifen
- combined oral contraception

Medications ADR that can cause menorrhagia
A
- warfarin
- other anticoagulants

OTC treatment
A
Tranexamic acid (anti-fibrinolytics)

Menorrhagia tranexamic dose
A
once heavy bleeding starts
- take TWO 500mg tabs TDS for max 4 days
- if needed QDS

Tranexamic Acid OTC Licensing
A
- women aged 18 to 45
- needs to have 21 to 35 day regular menstrual cycle
- no more than 3 day variabiliy

Side effects of Tranexamic Acid
A
- Nausea
- vomiting
- diarrhoea
- visual disturbances
- thromboembolic

Tranexamic acid contraindications
A
- hypersensitivity
- haematuria
- renal failure

Menorrhagia advice
A
- change towel once every 6 hours
- change tampons once ever 2 hours

If tampons are left longer than recommended what can occur
A
toxic-shock syndrome (infection due to tampon)
(can cause organ failure and death)
