Q

What is the purpose of section 2 of the TSR (safety limits)?

A

The safety envelope that the ATR must be operated within. Violations of limits require the plant to be placed in a safe condition and require analysis to resume operation.

16
Q

What is the purpose of AC 5.4.3/4?

A

Violation of TSR and response to TSR violations.

17
Q

What is the purpose of AC 5.7.11?

A

Requires an ESAP for each experiment.

18
Q

Lessons learned ATR EFIS Valve failure?

A

EFIS time response testing being performed.
Portion of testing included valve actuation with n2 bottle backup system.
EFIS valves were actuated for RCR, one valve shut and other remained open with hissing sound.
I&C tech and operator returned to area found LCV-1-7A/B in OPEN position. No pressure in n2 bottle.
Operations went outside of procedure, reset the solenoids and the valve fractured.

19
Q

Lessons learned from PT-10-6 calibration?

A

PT-10-6 indicated pressure dropped below min level during maintenance activities. Calibration was performed, and alarm received. Returned to normal after vent was shut. Wrong alternate isolated PT to meet LCO.
