Inguinal Canal Flashcards
Boundaries
- Inguinal canal: 3-5 cm tunnel they abdominal wall along inguinal ligament
- Under 3 arches of Ab. Wall muscle
- Deep ring: spermatic cord thru endoabdominal fascia
- Superficial ring: cord thru external oblique
- Inguinal ligament: thanked terminal external oblique fascia from ASIS to pubic tubercle
- Lacunar ligament: attaches inguinal ligament to pectinate line
Fascial layers surrounding testes
- Parietal peritoneum -> tunica vaginalis
A. Serous membrane
B. Attaches cremasteric fascia - Endoabdominal fascia -> inner spermatic fascia
- Transversus abdominis -> nothing
A. Goes over canal, not part of it
B. Ends at inner ring - Internal oblique -> middle cremasteric fascia and muscle fibers
A. Cremaster muscle: reflex and temp regulation - External oblique -> external spermatic fascia
- Subcutaneous deep membranous fascia (Scarpa’s fascia) -> dartos fascia and muscle
A. Autonomic control
B. Temp regulation
Inguinal canal contents
- Spermatic cord
A. Vas deferens: sperm pathway
B. Vasculature- Testicular artery: off aorta (L2), below renal arteries
- Cremasteric artery: from inf. Epigastric
- Papiniform venous plexus
A. Right draining IVC
B. Left into left renal
C. Important for heat dissipation
C. Nerves - Genital branch of genitofemoral n.: innervates cremaster muscle
- Sympathetics
A. Testicular plexus: w/ testicular artery and vas deferens (sympathetic chain T11-L1)
B. Dartos: sympathetic n. Runs w/ ilioinguinal and post scrotal n. (Outside spermatic cord) - Parasympathetics: from pelvic splanchnic S2-4
D. Lymphatic drainage: testes (para-aortic nodes)
E. Vestige of process is vaginalis if not obliterated
F. Testes
Gubernaculum
Ligament-like primordia
- Attaches dev. Testes/ovaries and scrotal/labia folds
- Course thru abdominal wall -> site of inguinal canal
Processes vaginalis
Finger-like projection formed as gubernaculum pulls testes/ovaries thru abdominal wall
1. Becomes tunica vaginalis
Inguinal canal dev in females
- Ovaries pulled by gubernaculum
- Stop before pelvic brim
- Gubernaculum -> ovarian lig superiority and round lig of uterus inferior
- Round lig thru inguinal canal -> labia majora
- Processes vaginalis -> canal of nuck
Hernia
Abdominal viscera protrudes thru fascia w/ inc. pressure
1. Direct: medial inguinal fossa (hasselbach triangle)
A. Borders
1. Inf. Epigastric a.
2. Inguinal lig.
3. Lateral rectus abdominus
B. Contains by external oblique or thru superficial ring
- Indirect (congenital): thru processus vaginalis w/in spermatic cord or canal of nuck
- Femoral: they femoral ring, along femoral vessels
Hydrocele and hematocele
Fluid/blood w/in layers of tunica vaginalis
Sebaceous cyst
Cyst w/in epidermis
1. Usually involve hair follicle
Spermatocele
Cyst w/in head of epididymis
1. Most common cyst in scrotum
Cryptorchidism
Undescended teste
- 3% full term babies
- 30% premies
- 80% descend w/in first year
Monorchism
One teste
Polyorchism
Extra testes
Ectopic testes
Lodged in abdominal cavity
Vasectomy
Vas deferens clipped and tied
