Definition of IBS
GI syndrome characterized by chronic abdominal pain and altered bowel habits in the absence of any known cause
IBS more common in males or females

Usually less than 45 yo
What is considered the main culprit in IBS?
Motility disorder…many treatments aimed at modifying the dominant motility pattern
What is visceral hypersensitivity?
Increased sensitivity to pressure and pain stimulus
Intestinal inflammation in IBS
Certain patients show increased inflammatory markers like TNF
Most common trigger of an IBS episode?
Psychological cuases
Corticotropin Releasing Factor (CRF)
possible mediator of IBS
Diagnostic criteria for IBS
Recurrent abdominal pain or discomfort for 3 consecutive days a month in the last 3 months with 2 or more of the following:
- improvement with defecation and/or
- onset associated with change in stool frequency
- onset assoc with change in stool appearance
IBS-C
Hard stool over 25% of stool
IBS-D
Loose stool over 25% of stool
Mixed IBS
Both loose and hard
IBS pts do NOT have Nocturnal diarrhea
KNOW
IBS diarrhea usually right after a meal

Red Flags in IBS
wt loss, blood, recent travel, older age, family history of cancer, antibiotic use, etc…
