what is the duty of candour?
openness, transparency and candour throughout the health care system - being open and honest when something goes wrong
- put matters right
- offer an apology
- explain fully and promptly what has happened and the short and long term effects
what must you do under duty of candour if something has gone wrong?

what triggers a statutory duty of candour in dentistry?
an increase in pt treatment e.g., #endo file
changes to the structure of the pts body e.g., wrong tooth xla
impairment of function which lasted for at least 28 days e.g., paraesthesia after hypochlorite incident
pain/ psychological harm that lasted at least 28 days e.g., accessed wrong tooth leaving pt with irreversible pulpitis

what triggers a statutory duty of candour?
- if the pt safety incident one that was ‘unintended or unexpected’ in the context of care provided?
- in the opinion of a registered health professional, did the incident appear to have resulted in, or could result in: death, permanent lessening of function
- is the harm related directly to pt safety incident rather than a natural course of the pts illness or underlying condition
what are the short term side effects of GP extrusion into the sinus?
discomfort, additional surgery required
what are the long terms of GP extrusion into the sinus?
possible chronic sinusitis
