what is degloving
separation of skin from underlying surfaces
what is laceration
where skin has burnt or torn (usually consequence of blunt trauma)
what is an incised wound
cause by sharp object in sharp trauma
what is seen in a nail fingertip injury
subungual haematoma- if pressure causing pain can do trephine surgery
nail may fall off but will grow back
in nail and nailbed injury why should you try and maintain the nail
as splint to maintain nail fold
if a nail is removed, why must the nail bed be repaired
as it determines how the nail grows back
how do you treat nail injuries
level 1 (soft tissue) and 2 (ST + nail)= dressing only
level 3 (+ bone)= repair nail bed and stabilise the bone
level 4 (proximal 1/3rd of phalanx) as above unless <5 mm of nail bed- albate
describe a boxers fracture
minimal displacement, no rotation, more distal
how do you treat a boxers fracture
neighbour strapping and early mobilisation
how do you unmask a rotational injury
closing fist will show that one finger wont bend
describe a mallet finger
when DIP bend and they cant straighten it themselves- seen in avulsion fractures
how do you manage a mallet finger
mallet splint
occasionally fixed with wire (if large displaced avulsion fracture) or dermatotendoesis in chronic cases
how do you treat a PIPjt dislocation
treat acutely (vital)
pull to reduce
buddy strap
what happens in delayed presentation of PIPjt dislocation
impossible to reduce
may require fusion
whats a bennetts fracture
fracture of the base of the first metacarpal bone which extends into the carpometacarpal (CMC) joint
