reversible block of H2 receptors - decr H+ secretion from parietal cells
use: peptic ulcer, gastritis, mild reflux
H2 blockers MOA
omeprazole, lansoprazole, esomeprazole, pantoprazole, dexlansoprazole
PPI’s
irrevers inhib H/K ATPase in stomach parietal cells
use: peptic ulcer, reflux, Zollinger Ellison
PPI’s MOA
bind to ulcer base - give phys protection & allow HCO3- secretion to re-establish pH gradient in mucus layer
use: ulcer healing, traveler’s diarrhea
bismuth, sucralfate
PGE1 analog, incr production & secretion of gastric mucus barrier, decr acid production
use: prevent NSAID peptic ulcers (NSAIDs block PGE1 production), maintain PDA, off label inducts labor (abortion)
Misoprostol
long acting somatostatin analog, inhib actions of vasoactive intestinal peptide
use: acute variceal bleeds, acromegaly, VIPoma, carcinoid tumors
Octreotide
magnesium hydroxide, magnesium citrate, polyethylene glycol, lactulose
osmotic laxatives
provide osmotic load to draw water into GI lumen
use: constipation, hepatic encephalopathy
osmotic laxatives MOA
lactulose treats hepatic encephalopathy
combo of sulfapyridine abx + 5ASA anti inflam
activated by colonic bacteria
use: UC, Crohn’s
sulfasalazine
5HT3 antagonist, lowers vagal stimul, powerful central acting anti emetic
use: post op vomiting, cancer chemo SE
ondansetron
D2 receptor antagon, incr resting tone + contractility + LES tone + motility but NO influence on colon transit time
use: diabetic & post surg gastroparesis, anti emetic
Metoclopramide (pro motility)
inhib gastric/pancreatic lipase - lowers breakdown & absorpt of dietary fats
use: wt loss
