request the attendance of police for possible illegal activity and wait for there arrival
Effecting entry what form should be completed?
Form 2416
To Effect an entry, form 2416 requires a signature before doing so to advice that damage maybe caused which the brigade are not liable for. When would be unable to get a signature.
If the person is under the age of 18
the child’s parent/guardian name and address should be recorded.
If the person is of an age or condition which requires supervision, the immediate
attendance of the police should be requested

If it is decided that No service is to be rendered, is there a need to record this on a form 2416?
YES
Where no service is rendered at a lockout incident, a Form 2416 is to be completed as a report of
the attendance. The person requesting the service is not required to sign in this instance. These
forms are also to be submitted to the relevant Borough Administration Team
Following a CBLD incident what form should be completed?
CBLD 1 form and CBLD-1 form and submit it to the Blue Light Collaboration mailbox (BLUELIGHTCOLLABORATION in the address book).
