What are the 2 classes of strokes?
ischemic, hemorrhagic
This is a clot that forms at a site of atherosclerotic plaque rupture
thrombotic ischemic stroke
What is the main sources of embolic strokes?
carotid artery clots, mural thrombi from afib, DVT in pt with ASD or PFO
What 3 things reduce the infarction size?
anticoagulation, thrombolytic therapy, endovascular therapy
What are some anticoagulation stroke preventions?
- agents affecting platelet fx; 2. vitamin K dependent thrombin inhibition; 2. factor Xa
What is the anticoagulation preferred drug for acute ischemic stroke?
low molecular weight heparin
Heparin inhibits thrombin indirectly by ?
enhancing antithrombin III
What are the 3 heparin toxicities?
hemorrhage, thrombocytopenia, bone loss
What are the 2 types of HIT and which one is associated w/ thrombosis?
Type I (benign) Type II (dangerous)- thrombosis
LMW Heparin preps.
Enoxaprin, dalteparin, tinzaparin
What is the therapeutic window for thrombolytic drug therapy?
2-6 hrs
What is the thrombolytic strategy?
activate plasminogen to become plasmin
What are the indications of thrombolytic drugs?
DVT, PE, arterial thromobosis, clotted shunts and catheters
What are the contraindications of thrombolytic drugs?
surgery w/in 10 days, GI bleeds, hx of HTN, active bleeding disorder, previous hemorrhagic stroke, pregnancy
