Distribution
A
- Focal=one single lesion
- Multifocal=several lesion (number)
- multifocal to coalescing= several lesions joining
- miliary= too many to count
- locally extensive= uniform, spreading
- segmental= in hollow organs or snakes
- diffuse= entire organ

Demarcation
A
- Well demarcated= abrupt and easily seen boundary
- poorly demarcated= blurred or not seen boundary

Contour
A
- Raised= something is added
- depressed= something is removed
- flat

Shape
A
What geometric figure does it resemble?

Color
A
Describe what color it has, if it varying or genuine

Size
A
Use something with standardized measurement, or actual measurements.
If more than one lesion= uniform or not

Texture
A
- amorphous= semisolid, unorganized, “spreadable”
- solid tissue= maintains shape, “unspreadable”

Consistency
A
How does it feel?
- gas= like bubble plastic, air trapped in tissue
- fluid= wet or squishy, like a water balloon
- fluid= looks or feels squishy, like a sponge
- soft= fluid rich, cell or stroma poor
- form= fluid poor, cell or stroma rich
- hard/gritty= cannot be cut

Fluids
A
- Measured in volume and type
- serous= clear
- serosanguinous= clear with blood tings
- serofibrous= cloudy with strands of fibrin
- chylous= milky white, lymph
- purulent= opaque, thin to thick, pus

Sound
A
- crepitant=sounds of popping air, bubble plastic
- sloshing= sound of fluid splashing
- hard= sound like a rock when banged on hard surface

Odor
A
- foul=rotting smell
- ammonia= usually means uremia
- apple cider= gastric hemorrhage or swallowed blood
- no odor
