Wk 2 Burns Flashcards
Standard burn fluid resuscitation
Parkland
4cc/kg/TBSA (total body surface area)
1/2 in first 8 hrs
other half in next 16 hrs
What does ascorbate do when added to IVF?
decreases overall damage due to burn
esp in organs
acts as free radical antioxidant
Three zones of burn
zone of coagulation
zone of stasis
zone of hyperemia
Why do you push IVF after burns?
prevent zone of coagulation from increasing in size and
increases size of zone of hyperemia
Average urine output of adult
0.5cc/kg/hr
4 degrees of burn and descriptions
1: red
2: blisters
3: painless
4: bone
Silvadene
- use
- complications
- topical antibacterial ointment for 1st/2nd degree burns
- transient neutropenia
composed of silver and sulfadiazine
Mafenide acetate (Sulfamylon)
- use
- complications
- topical antibacterial burn ointment
- metabolic acidosis/carbonic anhydrase inhibitor
Silver nitrate
- use
- complications
- topical antibacterial burn ointment
- leeches out Na+, K+, Ca2+, Cl-
Escharotomy
cautery lines cut to release pressure from circumferential burns
Biggest concern with victim of electrical shock
hydration–proteins damaged in shock will be flushed out giving urine “tea” color
urine output should be > 100 cc/hr
4 degrees of frostbite
1: erythema
2: blisters
3: sub-Q damage
4: gangrene
Rapid rewarm temp for frostbite
104 F
Should you neutralize acid/base burns?
NO the heat from the Rx will cause a thermal burn
